Registration Process Instructions

STEP 1: Download and print the Student Medical Form if you have not filled one out already.

STEP 2: Submit the medical form.
  • Submit in Person at Sunday Morning, Tuesday or Wednesday night, or drop it off at the office
  • Submit via Fax: (603) 883-5814
  • Submit via Mail: Trinity Baptist Church, Student Ministries, 33 Lund Rd, Nashua, NH 03060

STEP 3: Submit payment on or before the specified date and be sure to let us know who it is for.
  • Make checks payable to Trinity Baptist Church or pay cash
